Mental Health Case Manager

Mental Health Case Managers coordinate comprehensive care plans for individuals with serious mental illness, substance use disorders, or co-occurring conditions, linking clients with clinical services, housing, benefits, employment, and social supports while monitoring treatment adherence and crisis prevention. They function as the central coordinator for complex clients receiving services from multiple providers and systems. Integrated care models and ACO frameworks have significantly expanded the need for mental health case management.
