Case Manager

Case Managers coordinate care plans for patients with complex health and social needs, working across healthcare providers, insurers, and community resources to ensure appropriate services and outcomes. They advocate for patient needs and monitor plan adherence. RN or social work licensure combined with case management experience is required.
