Patient Navigator

Patient Navigators help patients and families navigate complex healthcare systems, overcoming barriers to care access such as insurance coverage gaps, language barriers, transportation challenges, and health literacy limitations. They coordinate care across multiple providers, connect patients with community resources, and track care plan adherence to improve health outcomes and reduce unnecessary emergency department utilization. This role is particularly important in oncology, chronic disease management, and vulnerable population programs.
